Many of our clients are simply looking to stabilize a current crisis in the home, but additionally we can serve while a client is located in an inpatient, skilled or community based setting. We work to collaborate with staff to coordinate an appropriate discharge based on your individual needs.

Additionally, Compass Care offers fully integrated, customized case management, which involves assessment, planning, care coordination, evaluation and advocacy for our clients. Whether you simply need an in town contact person for your loved one or you are looking for a licensed professional to assist you with frequent on-site assessments and plan modifications, we are able to assist through hourly consultations or our complete case management program that includes monthly care planning and evaluations.
Every one of our ongoing case management plans starts off at a face to face evaluation with a licensed professional, and is developed with all current and potential needs in mind. We strive to make an uncertain time feel more stable as we develop a plan that feels just right for you.

Compass Care is the only San Diego firm to provide not only comprehensive case management services, but clinically based, concierge level placement services to the families and seniors of our community. Our staff has an extensive clinical background from inpatient through hospice, as well as hands on experience in the senior living communities throughout the county.
Our placement process involves a thorough evaluation of your needs while reviewing your social, emotional, financial, and locational needs so we can identify the ideal community for you. We will discuss possible placements, schedule tours, visit communities together, and assist you with all aspects of your move through moving day and beyond.

Education and support is the foundation of Compass Care’s vision, with that in mind we provide consulting and educational services to community partners, senior living communities, professionals and caregivers of San Diego county. Our areas of focus include Dementia education with a specialty in behavioral and medication management, geriatric specific medical interventions, appropriate transitions through complex levels of care in the community, and hands on education of staff, families and caregivers.
